Your understanding of the counseling process is important to us. Our understanding of the purpose of your visit is equally important. All of the paperwork you will need is available below. You have the option of downloading the paperwork and filling it out before you come or we will have the forms sitting out for you in the waiting area. The forms take about 15 minutes to fill out, so please arrive early if you plan to fill them out in the office.
Your personal information is protected by state and federal laws. On your first visit your counselor will review the forms with you to be sure that you understand your rights and responsibilities as a client of Great Oaks as well as your counselor's responsibilities to you. The biographical form will help your counselor become aware of the personal goals and issues that have led you to seek counseling.

Fees & Insurance
The fees of our licensed therapists, depend on their credentials and years of experience. We request payment at the beginning of each session. Please ask your therapist, if they are affiliated with your insurance company. If not and you wish to use your out-of-network benefits, we ask that you have your insurance company reimburse you. If you need a diagnostic receipt, please ask you therapist and they will be happy to provide one.
A schedule of payment for services or a reduced rate because of unusual financial need can be negotiated with your therapist, but the final decision is at the sole discretion of the therapist.
Payments of cash, check, credit or debit cards are accepted.
